competition score card

Hi Guys im running a local community shore competition and wondering if any of you's have a score card lying around the house you wouldn't mind scanning or even point me in the direction of a link where i may be able to download one myself, i have searched the web and cant seem to find any score cards anywhere.....

Total species boat/shore: 45

If you can open them in excel and word you will be able to edit them to suit the venue.
